Job summary

UnitedHealth Group is seeking a Wellness Coordinator in Missouri to provide care coordination and administrative support for behavioral health and substance use services.

You will engage members, screen for gaps in care, and coordinate with clinical teams to connect individuals with needed resources. Strong PHI security and CST‑time availability are essential for this remote/telecommute role.

Qualifications

• High School Diploma.
• 2+ years of behavioral health and case management experience.
• 1+ years of computer proficiency experience.
• 1+ years of dual diagnosis experience with mental health and substance abuse.
• 1+ years of experience handling sensitive issues with members and providers in a confidential manner.
• Access to a designated quiet workspace in your home with the ability to secure PHI.
• Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
• Ability to work a 40-hour work week in the time zone related to the work (CST).

Responsibilities

• Wellness Coordinators work in an inbound and outbound member queue.
• Documentation in multiple electronic documentation systems.
• Coordinate information to clinical team partners.
• Provide behavioral health benefit education.
• Contact members to screen for and identity gaps in care.
• Provide network referrals and appropriate community resources to help the member engage with mental health and substance use treatment.
• Administrative support.
• Performs other duties as assigned.
