Job summary

UnitedHealth Group seeks a patient-centered care coordinator to support at-risk individuals, coordinating care plans and monitoring adherence. You will liaise with primary care and behavioral health teams and document observations in the electronic medical record.

The role requires a high school diploma, EMR experience, and solid Microsoft Office skills, with one year in social services or behavioral health preferred. Office-based with some onsite presence as needed.

Qualifications

• High School Diploma or GED required.
• 1+ years of experience in a social services or behavioral health setting.
• 1+ years of experience with electronic medical records (EMR).
• Intermediate proficiency with Microsoft Office (Word and Excel).

Responsibilities

• Facilitates the process of care for at risk patients, coordinates an accessible plan of care and tracks adherence.
• Serves as a liaison between the primary care team, behavioral health clinicians and patient and/or patient's family.
• Documents processes and standard work; identifies opportunities to improve quality and supports care improvement activities.
• Assists with cross-coverage and ensures all documentation is entered into the electronic medical record.
