Job summary

UnitedHealth Group is seeking a Wellness Coordinator to provide care coordination and administrative support for behavioral health and substance use needs. You will engage members, identify gaps in care, and help develop individualized care plans while supporting the Behavioral Health Case Management team.

The role involves inbound/outbound member interactions, documentation, and coordination with clinical partners.

Qualifications

• High School Diploma required.
• 2+ years experience in behavioral health and case management.
• 1+ years computer proficiency.
• 1+ years dual diagnosis experience with mental health and substance use.
• Experience handling sensitive information confidentially.
• Must have a designated quiet home workspace with PHI security.
• Must have reliable high-speed internet and CST-friendly hours.

Responsibilities

• Work in an inbound and outbound member queue
• Document in multiple electronic systems
• Coordinate information with clinical team partners
• Educate on behavioral health benefits
• Contact members to screen for gaps in care
• Provide network referrals and community resources
• Provide administrative support
• Other duties as assigned
