Job summary

Elite Care Management, Inc. in Bloomingdale, IL is seeking a Part-time Skilled Nurse (RN or LPN) to provide one-on-one in-home care for a single patient per shift, assessing needs and delivering care to promote comfort and quality of life.

Shifts: 7am–7pm Sunday and various weekdays, 7pm–7am every other Saturday. Includes medication administration and care planning. Paid orientation, weekly pay, benefits, and 24/7 on-call support.

Qualifications

• Active Illinois RN or LPN license.
• Current CPR Card (card must be valid).
• TB test results (2-step) or chest x-ray within 12 months; or QuantiFERON gold within 5 years.
• Driver's License required.
• Social Security Card required.

Responsibilities

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of changes.
• Follow organizational nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage and direct patient care according to the patient care plan and update as needed.
• Administer medications and treatments as prescribed, and monitor effectiveness.
• Intervene and report changes in patient condition to physician or supervisor.
