Job summary

Elite Care Management, Inc. is seeking a part-time skilled nurse (RN or LPN) to provide one-on-one home health care in Riverwoods. You will care for a single patient per shift, support daily living activities, and follow the patient care plan with close communication to the family and supervisor.

The role requires licensure as RN or LPN, valid CPR, TB test or equivalent, and a driver’s license. Competitive salary, benefits, and weekly pay are offered, with training and ongoing on-call support.

Qualifications

• Strong communication skills and ability to document observations.

Responsibilities

• Observe patient, document conditions and reactions, and report updates to supervisor.
• Follow nursing policies and procedures and maintain care plans.
• Administer medications and treatments as prescribed and monitor effectiveness.
• Communicate clearly with patients, families, and healthcare team.

• 7am - 7pm - Tuesday's and Thursday's

You will get to know your patients and their families and learn about the needs they may have in their home. Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift. As a Skilled Nurse (Registered Nurse or License Practical Nurse)

You will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
