Elite Care Management, Inc. is urgently seeking a part-time home health nurse to provide one-on-one care to patients in their homes in Riverwoods, IL. Each shift focuses on caring for a single patient, ensuring comfort and well-being with a personalized care plan.

You will observe, assess, and support daily living activities, administering medications as prescribed. The role requires an active RN or LPN license, ability to work a 7am–7pm shift on Tuesdays and Thursdays, and strong communication

Qualifications

• Observe patient, record significant conditions and reactions.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage patient care according to the care plan and update as needed.
• Administer medications and treatments as prescribed and document effectiveness.
• Intervene as needed and report changes to physician or supervisor.

• 7am - 7pm - Tuesday's and Thursday's

Join the world of HOME HEALTH CARE, where you can provide ONE-ON-ONE care to patients in the comfort of their home. You will work with ONE PATIENT for your complete shift. You will get to know your patients and their families and learn about the needs they may have in their home. Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift. As a Skilled Nurse (Registered Nurse or License Practical Nurse)

You will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.
