Elite Care Management, Inc. is seeking a part-time home health nurse to provide one-on-one care in patients' homes in Bloomingdale. You will care for one patient per shift, observe health status, administer meds, and support daily living activities.

The role requires RN or LPN licensure and a flexible schedule. Shifts include 7am–7pm on Saturdays, various weekdays/weekends, and 7pm–7am every other Saturday, with competitive hourly pay and full benefits such as medical, dental, vision, 401K, PTO,

PATIENT LOCATION - BLOOMINGDALE

Shifts:

• 7am - 7pm - Saturday
• 7am - 7pm - Various Weekdays & Weekends
• 7pm - 7am - e/o Saturday

Pay Range:

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
