Job summary

Elite Care Management, Inc. seeks a part-time Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care in Bloomingdale.

You will care for a single patient per shift, supporting daily living activities and comfort, and implementing the patient care plan under supervision. Responsibilities include assessing conditions, administering medications as prescribed, documenting changes, and communicating with the supervising physician.

Qualifications

• RN licensed or LPN with active license
• CPR certification current
• TB test within 12 months or chest X-ray within 12 months

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
