Elite Care Management, Inc. is seeking a part-time Home Health Nurse to provide one-on-one in-home care to patients in Bloomingdale, IL. You will work with a single patient per shift, coordinating care and ensuring comfort and well-being.

Licensed RN or LPN in Illinois required, with home health experience preferred. Shifts include 7am–7pm and 7pm–7am, with a competitive salary and full benefits package offered.

Responsibilities

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
