Elite Care Management, Inc. is seeking a part-time Home Health Nurse to provide one-on-one care to patients in their home in Bloomingdale. You will work with a single patient per shift, gaining insight into their needs and coordinating care plans.

The role requires an active RN or LPN license, CPR certification, and reliable transportation. A competitive benefits package and flexible scheduling are offered.

Qualifications

• Active RN or LPN license required.
• Current CPR certification.
• TB test results within required timeframe or chest x-ray within last 12 months.

Responsabilités

• Observe patient, record significant conditions and notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage and direct patient care per the patient care plan and update as needed.
• Administer medications and treatments as prescribed and document effectiveness.
• Intervene as needed in response to patient condition changes and report to physician or supervisor.

Shifts:

• 7am - 7pm - Saturday
• 7am - 7pm - Various Weekdays & Weekends
• 7pm - 7am - e/o Saturday

Pay Range:

• $35 - $40 / hour

Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift. As a Skilled Nurse(Registered Nurse or License Practical Nurse)

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
