Job summary

Elite Care Management, Inc. is seeking a part-time Home Health Nurse to provide one-on-one care to patients in their homes. You will work with a single patient per shift, observing health status, administering medications, and coordinating with the care team.

Valid RN or LPN license and CPR are required. A driver's license and reliable transportation are preferred. Competitive pay, benefits, and weekly direct deposit are offered.

Qualifications

• Experience in home health care preferred.
• Ability to document patient care clearly and promptly.
• Strong communication skills with patients and families.

Responsibilities

• Observe patient and document conditions and reactions; notify supervisor or physician as needed.
• Follow nursing policies and procedures accurately.
• Communicate effectively, both verbally and in writing.
• Manage and direct patient care per care plan and update as needed.
• Administer medications and treatments; monitor effectiveness and adjust as directed.

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
