Job summary

Elite Care Management, Inc. is seeking a part-time nurse (RN or LPN) to provide one-on-one in-home care in Bloomingdale, IL. You will work with a single patient per shift, following the care plan, administering medications, and coordinating with family and supervisors to support daily living and comfort.

Shifts include daytime, weekends, and some evenings with a competitive hourly rate of $35–$40. We offer paid orientation, weekly pay, comprehensive benefits, and 24/7 on-call support.

Qualifications

• Valid RN or LPN license in Illinois.
• Driver's license and auto insurance card.
• Current CPR certification.
• TB test or QuantiFERON within the required timeframe.
• Ability to provide one-on-one in-home care.

Responsibilities

• Observe patient, document conditions, and notify supervisor of changes.
• Follow patient care plan and administer medications.
• Demonstrate strong communication with patients and families.
• Manage and direct in-home patient care as needed.
• Document interventions and monitor effectiveness.
• Respond promptly to changes in patient condition.

• 7am - 7pm - Saturday
• 7am - 7pm - Various Weekdays & Weekends
• 7pm - 7am - e/o Saturday

Pay Range:

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents
• Demonstrate understanding organizational and nursing policies and procedures
• Demonstrate strong communication skills, both spoken and written
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor
