Job summary

Elite Care Management, Inc. is seeking a part-time Registered Nurse or Licensed Practical Nurse to provide one-on-one in-home care in Riverwoods, IL. You will support patients with daily living activities, comfort, and overall well-being during your 12-hour shift on Tuesdays and Thursdays.

Qualifications

• Licensed RN or LPN in Illinois (active).
• Ability to provide one-on-one home health care.
• Current CPR and TB test (or equivalent) on file.

Responsibilities

• Observe patient, record significant conditions, and notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication, both spoken and written.
• Manage and direct patient care per the care plan and update as needed.
• Administer medications and treatments as prescribed and monitor effectiveness.
• Intervene as needed and report changes to physician or supervisor.

Shifts:

• 7am - 7pm - Tuesday's and Thursday's

Join the world of HOME HEALTH CARE, where you can provide ONE-ON-ONE care to patients in the comfort of their home. You will work with ONE PATIENT for your complete shift. You will get to know your patients and their families and learn about the needs they may have in their home. Then you will plan, organize, and assist with home health care service with your focus on just one patient for your shift. As a Skilled Nurse (Registered Nurse or License Practical Nurse)

You will assist patients with daily living activities, ensure their comfort, and support their overall well-being to enhance their quality of life.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
