Job summary

Transitions Hospice in Marshall, IL is seeking a Registered Nurse (RN) Case Manager to coordinate care for patients and oversee the interdisciplinary team. This full-time role runs 8 AM–8 PM with rotating 4-day and 3-day weeks, covering Marshall and surrounding areas.

You will administer medications, document care, and communicate with families and physicians while participating in on-call rotations. Join a compassionate team delivering home-based hospice care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.
