Transitions Hospice is seeking an RN Case Manager to coordinate care for patients in the Crown Point, IN area. You will be the clinical anchor for patients and families, guiding the interdisciplinary team and ensuring care plans align with physician orders.

You will conduct assessments, build personalized plans, manage medications, and maintain accurate documentation while participating in on-call and holiday rotation. Competitive compensation and strong team support are offered.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One year+ Hospice, home health or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsabilités

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.

Coverage Area: Jasper, Lake Newton , Porter, Laporte and surrounding counties Salary: $80,000 - $90,000 / year Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation Back-up on call and Holiday rotation
