Transitions Hospice in Champaign, IL is seeking a Registered Nurse Case Manager to serve as the clinical anchor for patients and families. You will coordinate care, guide the interdisciplinary team, and support families in end-of-life care with compassion.

The role requires an active RN license in Illinois, a nursing degree, and at least 1 year of hospice/home health experience. Benefits include full health coverage, 401(k) with match, auto allowance, PTO, and more.

Responsabilidades

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
