Job summary

Transitions Hospice in Oak Park, IL is seeking a Registered Nurse (RN) Case Manager to deliver compassionate hospice care across Oak Park, Cicero, North side Chicago and surrounding areas. You will be the clinical anchor, coordinating care with the interdisciplinary team and guiding care plans with physicians.

The role requires an active RN license, a nursing degree, and 1+ year hospice/home health experience.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families

Responsibilities

• Coordinate care as the clinical anchor for patients and families.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to feel confident and supported.
• Participate in on-call rotation per team policy

Salary: $85,000 - $95,000 / year

Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation (on-call and rotating holiday requirements)
