Job summary

Transitions Hospice in Naperville, IL seeks an experienced Registered Nurse Case Manager to coordinate patient care and lead the interdisciplinary team. The role focuses on comprehensive assessments, care planning, medication administration, and accurate documentation to support dignified end-of-life care.

You will educate families, coordinate with physicians, and participate in on-call rotations, working a schedule that alternates four- and three-day weeks with weekend coverage and rotating

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.

Salary: $85,000 - $95,000 / year

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation include rotating holidays
