Transitions Hospice in Bay City, MI is seeking a Registered Nurse Case Manager (RN) for a full-time role. You will coordinate care, lead the interdisciplinary team, and ensure compassionate, patient- and family-centered hospice services in home settings.

The role requires an active Michigan RN license and 1+ year hospice or home health experience; strong communication and independent practice are essential. Competitive pay and generous benefits are offered.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy

Coverage Area: Great Lakes, Bay Region, and surrounding areas

Salary: $40.00 - $46.00 / hour

Schedule: Monday - Friday (8am -5pm) with back-up on call requirements
