Job summary

Transitions Hospice in Elk Grove Village, IL is seeking a Registered Nurse to lead patient care in home-based hospice settings. You will provide compassionate, clinical support to patients and families, coordinate interdisciplinary care, and ensure comfort and dignity at end of life.

This full-time role requires 8 AM–8 PM shifts with alternating 4-day and 3-day weeks, on-call coverage, and weekend rotation.

Qualifications

• Must hold an active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and ability to work independently in the field
• Excellent communication skills across care teams and families

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy

Salary: $85,000 - $95,000 / year
