Transitions Hospice in Paxton, IL is seeking an experienced RN Case Manager to coordinate patient care, guide the interdisciplinary team, and respond when it matters most. You will be the clinical anchor for your patients, ensuring care plans align with physician orders and family needs.

You will conduct comprehensive assessments, build personalized plans, administer medications, maintain accurate documentation, educate families, and participate in on-call rotations to support 24/7 care at home.

Qualificações

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families

Coverage Area: Ford, Iroquois and surrounding counties
Salary: $84,000 - $94,000 / year
Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation w/ on-call rotation
