Job summary

Transitions Hospice - Michigan is seeking an RN Case Manager to serve as the clinical anchor for patients and families across the Great Lakes region, coordinating care with the interdisciplinary team.

You will conduct ongoing assessments, develop personalized care plans, administer physician-ordered treatments, ensure accurate documentation, and educate families. Participation in on-call rotation is required to cover patient needs.

Qualifications

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing
• One year Hospice, home health or related experience preferred
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and ability to work independently in the field
• Excellent communication skills across care teams and families

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient’s evolving needs
• Build and adjust personalized care plans in collaboration with physicians and care teams
• Administer medications and treatments per physician orders
• Keep clinical documentation accurate, timely, and meaningful
• Serve as the communication bridge between families, physicians, and community health partners
• Educate patients and caregivers so they feel confident and supported
• Participate in on-call rotation per team policy

Schedule:Mon-Friday (8am -5pm) with back-up on call requirements

Pay Range:$40.00 - $46.00 am hour
