Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families in the Mattoon/Charleston area. You will coordinate care, guide the interdisciplinary team, and respond to needs with on-call rotations.

The role requires an active RN license in the state and nursing degree, with preferred hospice/home health experience. Competitive pay and comprehensive benefits support your important work.

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsabilités

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy

Coverage Area: Mattoon / Charleston and surrounding areas

Salary: $70,000 - $85,000 / year

Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation
