Job summary

Transitions Hospice seeks a Registered Nurse (RN) Case Manager in Charleston, IL. You will coordinate patient care, guide the interdisciplinary team, and provide compassionate end-of-life support in home settings.

The role covers Mattoon/Charleston and surrounding areas, with a schedule of 8 AM–8 PM in a rotating 4-day/3-day week including weekends. Requirements include an active RN license in Illinois, graduation from an accredited nursing program, and Hospice/home health experience preferred.

Qualifications

• Active RN license in Illinois or the state of employment.
• Graduate of an accredited nursing program.
• Hospice, home health or related experience preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication across care teams and families.

Responsibilities

• Coordinate care as the clinical anchor for patients and families.
• Guide the interdisciplinary team and show up when it matters.
• Conduct comprehensive ongoing patient assessments.
• Build and adjust personalized care plans with physicians.
• Administer medications and treatments per orders.
• Keep clinical documentation accurate and timely.
• Serve as the communication bridge between families and physicians.
• Educate patients and caregivers for confidence and support.
• Participate in on-call rotation per team policy.
