Transitions Hospice is seeking a skilled Registered Nurse Case Manager in Columbus, OH. The role focuses on coordinating care for patients in New Albany and surrounding areas, guiding the interdisciplinary team, and ensuring high-quality, compassionate end-of-life care.

The RN will conduct assessments, develop personalized care plans, administer physician-ordered treatments, maintain precise documentation, and educate families to support informed decisions.

Formación

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related experience preferred.

Responsabilidades

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Skills, Knowledge and Expertise

What You Bring

• Active RN license in the state of employment
