Job summary

Transitions Hospice in New Albany, OH is seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care and lead the interdisciplinary team in the field.

You will conduct comprehensive assessments, develop personalized care plans with physicians and care teams, administer medications as ordered, maintain accurate documentation, and educate families to support home-based end-of-life care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers so they feel confident and supported.
• Participate in on-call rotation per team policy.

Schedule: M-F; 8 hour shifts (on call rotating)
