Job summary

Transitions Hospice in Crown Point, IN is seeking an experienced Registered Nurse (RN) Case Manager to coordinate hospice care for Jasper, Lake Newton, Porter, Laporte and surrounding counties.

This full-time role offers $80,000–$90,000 per year, 8 AM–8 PM scheduling with alternating 4-day and 3-day weeks, on-call and holiday rotation, and a team-oriented environment that honors dignity, comfort, and the moments that matter.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.

What You’ll Do
