Job summary

Transitions Hospice in Naperville, IL is hiring an RN Case Manager to coordinate patient care and guide the interdisciplinary team. You’ll act as the clinical anchor, ensuring timely documentation, medication administration, and clear communication with families and physicians.

The role requires an active RN license in the state, a nursing degree, and strong bedside judgment. Hospice or home health experience is preferred, with a dedication to compassionate, patient-centered care.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One year+ Hospice, home health or related experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.
