Job summary

Elite Care Management, Inc. is urgently hiring Home Health Nurses (RN or LPN) in Illinois. You will provide one-on-one care to a single patient in their home, managing daily activities, medications, and comfort, across full-time or part-time shifts. Sign-on bonuses apply and comprehensive benefits are offered.

Shifts include 6am–6pm, with PRN options; pay ranges from $35 to $40 per hour, with weekly pay and a full benefits package. A valid RN or LPN license and patient-focused care are required.

Qualifications

• Licensed RN or LPN in good standing.
• Ability to provide one-on-one home health care.
• Strong communication and documentation skills.
• Ability to manage patient care per care plan.

Responsibilities

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
