Job summary

Elite Care Management, Inc. is hiring skilled nurses to provide one-on-one home health care for a single patient per shift, with 6am–6pm coverage and possibilities for PRN weekdays/weekends. Join a team focused on comfort, safety, and personalized care in the patient’s home.

Responsibilities

• Observe patient and record significant conditions; notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Manage and direct patient care per the plan and update as needed.
• Administer medications and document effectiveness of interventions.
• Intervene in response to patient condition changes and report to physician or supervisor.

Pay Range:

• $35 - $40 / hour

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
