Fairview Health Services is seeking a Referral Specialist to manage patient scheduling, coordinate visits, and handle communications with patients via MyChart and phone. The role focuses on timely appointments, coordination with referring physicians, and ensuring smooth discharge follow-ups within the requested time frame.

The position requires experience in healthcare scheduling and strong customer service skills, with a Monday–Friday schedule (8:30 AM – 5:00 PM).

Qualifications

• 1 year experience in patient scheduling and registration in a healthcare setting
• 2 years years of customer service experience.

Responsabilités

• Makes outbound call attempt to patient following the Referral Specialist standard work.
• Uses system protocols to schedule the patient and support payment collection.
• Escalates clinic access barriers to ensure patients are seen within the recommended time frame.
• Ensures patients obtain appointments with the appropriate specialty.
• Manages hospital discharge orders and coordinates tests or procedures as indicated.
• Manages MyChart messages and online appointment requests.
