Job summary

Fairview Health Services in Saint Paul, MN, is seeking a driven Care Coordinator to join the Fairview Clinic Care Coordination team. The role supports complex scheduling, preventative care, and transitional care for patient panels.

Hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., 1.0 FTE, with clerical and EMR duties, calendar management, and coordination with physicians, RN care coordinators, and community partners to ensure seamless patient care.

Qualifications

• Basic health care insurance coverage knowledge.
• 2 years of Healthcare experience
• Use of EMR.

Responsibilities

• Completes the Care Coordination process for their panel patients.
• Liaison between clinic and community partners for shared initiatives, and long term collaborations.
• Facilitates complex health referrals.
• Promotes team environment within department, other site departments, and other clinics by acting as the Care Coordination subject matter expert.
