Qualifications

• Associate's degree required; bachelor's preferred in human services or related field.
• Strong knowledge of community resources and systems.
• Ability to travel within the community with reliable transportation.

• Coordinate and deliver recovery-oriented case management within a multidisciplinary team.
• Conduct comprehensive assessments and develop individualized care plans.
