Job summary

The Providence Center in Providence, RI seeks a Case Manager ACT II for the CCBHC program. You will join a multidisciplinary team to deliver person-centered, recovery-oriented case management with in-person and community-based support designed to reduce symptoms and promote independence.

Your role includes comprehensive assessments, care-planning, and connecting clients to housing, healthcare, education, employment, and other community resources, with ongoing monitoring and crisis intervention

Qualifications

• Associate's Degree required; Bachelor's degree preferred in Human Services or related field.
• Experience in human services is preferred.
• Valid driver's license and insured automobile are required.
• Strong knowledge of community resources in housing, education, employment, healthcare.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Conduct comprehensive assessments to identify client needs related to housing, education, employment, and healthcare.
• Develop and implement individualized care plans with clients and families as appropriate.
• Connect clients to community resources and provide ongoing monitoring and crisis intervention as needed.
