Job summary

The Providence Center seeks a Case Manager for the Pawtucket Health Home Team to coordinate services and support independence for clients with mental health and substance use challenges. This role involves person-centered, recovery-oriented case management delivered within a multidisciplinary team, including in-person and community-based support.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred (Field of Study: Human Services or related).
• Experience in Human Services field preferred.
• Must have valid driver's license, registration, current inspection sticker and insured auto.
• Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.
• Excellent organizational, problem-solving, and time management skills.
• Ability to travel within the community with reliable transportation.
• Bilingual preferred. For positions designated as bilingual, proficiency in the required language is mandatory.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Provide services to assigned caseload, including in-person contacts in office and community settings.
• Conduct comprehensive assessments to identify client needs related to housing, education, employment, financial stability, healthcare, and resources.
• Develop, present and implement individualized care plans with the care team, clients and families as appropriate.
• Connect clients to community resources and social services; provide ongoing monitoring and follow-up.
