Job summary

The Providence Center is seeking a Case Manager for the Pawtucket Health Home Team to coordinate services for clients, reduce psychiatric and substance use symptoms, and promote independence.

You will deliver person-centered, recovery-oriented case management within a multidisciplinary team, conduct assessments, develop care plans, link clients to housing, healthcare, education, employment, and community resources, and monitor progress in community settings.

Qualifications

• Associate's degree required; Field of study: Human Services or related.
• Bachelor's degree preferred.
• Experience in Human Services field preferred.
• Must have valid driver’s license, registration, current inspection sticker and insured auto.
• Bilingual preferred; for bilingual roles language assessment within 30 days.

Responsibilities

• Coordinate and deliver recovery-oriented case management as part of a multidisciplinary team.
• Conduct comprehensive assessments to identify client needs (housing, education, employment, healthcare).
• Develop, present and implement individualized care plans with clients and care team.
• Connect clients to housing, healthcare, education, employment, and community resources.
• Provide ongoing monitoring and crisis intervention as needed to ensure progress.
• Maintain accurate documentation and ensure compliance with program standards and regulations.
• Attend team meetings, case reviews, and community outreach activities.
• Transport clients to appointments and activities as required.
