• Experience in behavioral health settings advantageous.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Work with care team to align case management with clients’ mental health treatment plans.
• Conduct comprehensive assessments addressing housing, education, employment, financial stability, healthcare, and community resources.
• Develop and implement individualized care plans with clients, families, and care team.
• Provide support counseling for mental health issues, substance use disorders, and medical conditions.
• Monitor and follow up to ensure client progress; implement crisis protocols when required.
• Assist clients with daily activities and life skills; supervise medication self-administration.
• Connect clients to community resources and advocate for access to services.
