Job summary

The Providence Center Case Manager Team 1B coordinates services to minimize stressors and reduce psychiatric and substance abuse symptoms, promoting independence. Qualifications

• Associate's Degree required; Bachelor's preferred; field: Human Services or related.
• Experience in Human Services field preferred.
• Must have valid driver's license, registration, current inspection, and insured auto.
• Strong knowledge of community resources and systems (housing, education, employment, healthcare).
• Excellent organizational, problem-solving, and time management skills.
• Ability to build rapport with diverse clients and partners.
• Cultural competence and commitment to equitable, inclusive services.
• Ability to travel within the community with reliable transportation.
• Bilingual preferred; bilingual role requirements may apply.

Responsibilities

• Coordinate and deliver case management as part of a multidisciplinary team.
• Provide services to assigned caseload ensuring in-person contacts in office and community settings.
• Conduct comprehensive assessments to identify client needs.
• Develop, present and implement individualized care plans with the care team.
• Connect clients to housing, healthcare, education, employment, and other resources.
• Provide ongoing monitoring and follow-up to support progress.
• Align case management with clients' mental health treatment plans.
• Identify and respond to crises, including risk assessment and intervention.
• Advise clients and families on navigating healthcare, education, and social services.
• Prepare reports for grants, audits, and program evaluation.
• Attend team meetings and case reviews; participate in agency initiatives.
• Conduct home visits, school meetings, and community outreach.
• Assist clients with daily activities and life skills, including medication management and budgeting.
• Maintain accurate records of client interactions and services provided.
• Ensure compliance with policies and regulatory requirements.
• Transport clients to appointments and activities as needed.
• Foster relationships with staff, clients, and community partners.
• Participate in trainings and case presentations.
