Job summary

The Providence Center is seeking a Center Case Manager for the ACT Engagement and Diversion Team. This role supports clients in reducing psychiatric and substance use symptoms, managing life stressors, and achieving independence, through person-centered, recovery-oriented case management in office and community settings.

You will participate in comprehensive assessments, develop individualized care plans, and connect clients to housing, healthcare, education, employment, and other resources.

Qualifications

• Associate's degree required; Bachelor's preferred in human services or related field.
• Experience in human services field preferred.
• Must have valid driver's license, vehicle, and insurance.
• Bilingual skills preferred; required language assessment for bilingual roles.

Responsibilities

• Coordinate and deliver recovery-oriented case management as part of a multidisciplinary team.
• Conduct comprehensive assessments to identify housing, education, employment, healthcare needs.
• Develop and implement individualized care plans with clients and families.
• Connect clients to housing, healthcare, education, employment, and community resources.
• Provide ongoing monitoring, crisis intervention, and follow-up to ensure progress.
