Job summary

The Providence Center in Rhode Island is seeking a Case Manager for ACT Engagement and Diversion Team 1A to support clients in reducing psychiatric and substance use symptoms, building independent living skills, and linking to housing, healthcare, and other services.

You will conduct assessments, develop care plans with the team, provide crisis intervention, and maintain detailed documentation while collaborating with partner agencies to promote recovery and comply with program standards.

Qualifications

• Associate's Degree required; Bachelor’s degree preferred in Human Services or related field.
• Experience in Human Services field preferred.
• Must have valid driver’s license and be able to travel within the community.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Conduct comprehensive assessments to identify client needs related to housing, education, employment, healthcare, and community resources.
• Develop and implement individualized care plans in collaboration with clients, families, and the care team.
