Job summary

The Providence Center seeks a Residential Case Manager to provide clinical case management within a residential setting for adults with complex behavioral health needs. You will coordinate with a multidisciplinary care team to assess, plan, implement and monitor individualized care plans and supports.

Role emphasizes person-centered services, collaboration with families and community partners, and ongoing crisis intervention when needed. Travel within the community may be required.

Qualifications

• High School diploma or GED.
• 1–3 years of experience in a behavioral health setting.
• Valid driver’s license and insured vehicle.
• Strong knowledge of housing, education, employment, healthcare resources.
• Excellent verbal and written communication; ability to collaborate with multidisciplinary teams.
• Cultural competence; commitment to equitable and inclusive services.
• Ability to travel within the community.
• Willingness to work flexible hours including evenings, weekends, holidays.

Responsibilities

• Coordinate person-centered, recovery-oriented case management with the care team.
• Align case management with clients’ mental health treatment plans.
• Conduct comprehensive assessments of housing, education, employment, finances, healthcare and community resources.
• Develop and implement individualized care plans with clients and families.
• Provide support and counseling for mental health, substance use, and medical issues.
• Monitor progress and escalate crises as needed using QMHP resources.
• Assist with daily activities, life skills, medication management and budgeting.
