Job summary

The Providence Center in Providence, RI seeks a dedicated Case Manager for the Harmony Team to coordinate services for clients and support recovery within a multidisciplinary setting.

Responsibilities include assessments, care planning, linking clients to housing, healthcare, education, employment and community resources, and crisis intervention when needed to promote independence and community functioning. Travel to client sites may be required.

Qualifications

• Associate's degree in human services or related field required; bachelor’s preferred.
• Experience in human services and ability to work with diverse clients.
• Valid driver’s license, registration, and auto insurance required.
• Proficient in documenting in electronic medical records and case notes.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Conduct assessments to identify housing, education, employment, healthcare, and community needs.
• Develop, present and implement individualized care plans with clients and families.
• Connect clients to housing, healthcare, transportation, and financial assistance programs.
• Provide ongoing monitoring, crisis intervention and risk management as needed.
• Maintain accurate case records and documentation for audits and program evaluation.
• Attend meetings, home visits and community outreach to support clients in natural environments.
• Travel within the community to meet clients and provide services as required.
• Collaborate with partner agencies and care teams to promote recovery and adherence to standards.
