Job summary

The Providence Center is seeking a Case Manager to coordinate recovery-oriented, person-centered services within a multidisciplinary team. You will support clients in housing access, healthcare navigation, education and employment linkages, and community resource connections.

Responsibilities include comprehensive assessments, care plan development, crisis monitoring, and collaboration with care teams to promote independence and community integration. Travel to community sites may be required.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred in Human Services or related field.
• Experience in human services field preferred.
• Valid driver's license, registration, current inspection sticker and insured auto.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Provide services to assigned caseload ensuring in-person contacts in office and community settings.
• Conduct comprehensive assessments to identify housing, education, employment, healthcare, and community resource needs.
• Develop, present and implement individualized care plans with care team and clients.
• Connect clients to housing, healthcare, education, employment, and community resources.
