Job summary

Harmony Health Group in Massachusetts is seeking a Case Manager to support the clinical team by coordinating patient care, recording communications, and guiding daily schedules in line with state and federal regulations.

As CM, you will perform assessments, document progress, facilitate discharge planning, coordinate with outside providers, and help connect clients to community resources while maintaining HIPAA compliance.

Qualifications

• 2+ years of experience with behavioral health clients.
• Experience in mental health or substance use services.
• Experience working on a multidisciplinary team.

Responsibilities

• Coordinate and implement patient-related activities.
• Maintain daily progress notes and communications.
• Assist discharge planning and community referrals.
• Facilitate treatment team and cross-department communications.
