Job summary

Enhabit Home Health & Hospice is seeking qualified clinicians to review and approve patient information during start of care, recertifications, resumption of care, or evaluation visits. You will collaborate with branch leadership to educate LPs on documentation to meet regulatory standards and provide focused, one-on-one training when needed.

Qualifications

• Graduate from an approved school of nursing or physical therapy with a valid license.
• CPR certification and state license in good standing are required.
• Experience in Medicare home health or hospice settings is preferred.
• Strong technology skills and ability to use mobile devices.
• In-office attendance is essential for supervision and teamwork.

Responsibilities

• Review and approve patient information during start of care, re-certification, resumption of care or evaluation visits.
• Partner with branch leadership to educate and train LPs on documentation to meet regulatory standards.
• Provide one-on-one training when necessary to ensure thorough understanding.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
