Job summary

Enhabit Home Health & Hospice seeks a qualified clinician to review and approve patient information at starts of care and recertifications, partnering with branch leaders to educate LP staff on documentation and regulatory changes.

The role emphasizes high-quality patient care, training LPs, and maintaining strong relationships with referral sources, patients and families in a busy home-health setting.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license as per state requirements.
• Minimum one year of experience in Medicare home health or hospice.
• CPR certification and valid state driver license with automobile liability insurance.
• In-office attendance is essential for supervision, teamwork and task completion.

Responsibilities

• Review and approve all patient information submitted by the Licensed Professional during starts of care, recertifications, resumption of care, or evaluations.
• Partner with branch leadership to provide ongoing education and training to LPs on documentation requirements.
• Provide in-depth training one-on-one when necessary to ensure regulatory compliance.
• Promote quality patient care and maintain stable relationships with referral sources, patients, caregivers and staff.
