Job summary

Enhabit Home Health & Hospice is seeking a qualified nurse or physical therapist to review and approve patient information at start of care, recertification, resumption of care, or evaluation visits. You will partner with branch leadership to provide ongoing education to LPs to ensure documentation meets regulatory standards and stays current with changes.

The role emphasizes promoting quality patient care, maintaining strong relationships with referral sources, patients, caregivers and staff,

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must hold and maintain a valid clinical license per state requirements.
• Minimum of one year of experience in Medicare home health or hospice.
• CPR certification and a valid state driver's license with insurance.
• Strong technology skills including Microsoft Office.

Responsibilities

• Review and approve all patient information submitted by LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to LPs on documentation and regulatory changes.
• Provide in-depth one-on-one training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
