Job summary

Enhabit Home Health & Hospice is seeking a qualified Licensed Professional to review patient information during start of care, recertifications, and evaluations. You will train LPs, ensure documentation meets regulatory standards, and support branch leaders in staff education.

The role requires licensure, CPR, at least one year of experience, and experience in Medicare home health or hospice settings, plus reliable transportation for on-site work.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including Microsoft Office.
• In-office attendance required for non-remote roles.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs on documentation and regulatory requirements.
• Provide one-on-one training to LPs as needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
