Job summary

Enhabit Home Health & Hospice is seeking qualified nursing or therapy professionals to join a team dedicated to delivering exceptional care in the home. You will review and approve patient information at start of care, recertification, and evaluation visits, collaborating with branch leadership to ensure compliant documentation.

The role emphasizes ongoing education, staff training, and strong collaboration with referral sources, patients, and caregivers.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license in state; CPR certification required.
• Minimum one year of experience.
• Medicare home health or hospice experience preferred.
• Valid state driver license and automobile liability insurance.
• Experience with Microsoft Office (Word and Excel).
• Knowledge of Homecare Homebase (HCHB) preferred.

Responsibilities

• Review and approve patient information for start of care, re-certification, resumption of care, or evaluation visits.
• Partner with branch leadership to train LPs and ensure documentation compliance.
• Promote quality patient care and maintain relationships with referral sources, patients, and caregivers.
