Job summary

Enhabit Home Health & Hospice is seeking a qualified nurse or physical therapist to review and approve patient information for start of care and ongoing care visits. You will train LPs on documentation and uphold regulatory standards while fostering strong relationships with referrals, patients, caregivers and staff.

A valid nursing or PT license, CPR, and a year of Home Health/Hospice experience are required. In-office attendance is essential to support supervision and teamwork.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Basic demonstrated technology skills, including operation of a mobile device.
• In-office attendance is essential (except designated full-time remote positions).

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation requirements.
• Provide one-on-one training to LPs when necessary.
• Promote quality patient care and maintain relations with referral sources, patients, caregivers and employees.
